Trinity Mother Frances Rehabilitation Hospital is a 74-bed freestanding acute rehabilitation hospital. Serving the Tyler and east Texas community since 1993, we are dedicated to the treatment of patients who require rehabilitation after serious illness or injury and are proud to be nationally recognized as a "Stroke Center of Excellence." Our goal is to return the patient to a healthy, fulfilling and independent lifestyle. We provide a smooth transition from the acute care setting to the post-acute setting, offering the full continuum of care ranging from inpatient rehab, outpatient therapy and home health services.

For over five years, Trinity Mother Frances Rehabilitation Hospital has consistently scored in the top tier of HealthSouth's Clinical Ranking Report (a comparison of 94 freestanding inpatient rehab hospitals). The Clinical Ranking Report measures clinical performance with regard to patient outcomes and includes but is not limited to:

o improved function of the patient
o improved independence of the patient
o percent of patients discharged back to acute care
o percent of patients discharged home
o patient satisfaction

Trinity Mother Frances Rehabilitation Hospital is the only rehabilitation hospital in east Texas to offer the AutoAmbulator. Created by HealthSouth, the AutoAmbulator is a sophisticated treadmill device using the therapeutic concept of body weight-supported ambulation and robotics to help patients with gait disorders.

Trinity Mother Frances Rehabilitation Hospital was the first facility in east Texas to initiate the use of Animal Assisted Therapy in patient treatment. For over 10 years, certified therapy animals from Therapet Animal Assisted Therapy Foundation, along with Therapet's trained volunteers, have assisted in the treatment of a wide variety of patients, improving patient strength, mobility and cognition while also helping to fill the "furry void" of the pets that patients may have at home.

TMF Rehabilitation Hospital Services

o Designated as a Stroke Center of Excellence every year since 2004 - one of only seven rehabilitation hospitals in the nation with this designation. The criteria for Center of Excellence distinction includes almost 70 indicators that must be met. A sampling includes:
o Specialized staff - Certified Rehabilitation Registered Nurses; Neuro-IFRAH trained therapists and nurses; specially trained Stroke Treatment Team with annual proficiency exam
o Specialized program - cognitive retraining; interdisciplinary bowel and bladder program; spasticity management; neuropsychological testing and treatment
o Family education - disease process information; coping strategies; reintegration into community; Stroke Support Group endorsed by the American Heart Association. More than 90 percent of our stroke survivors are discharged home.
o The only Stroke Support Group in the Tyler area, endorsed by the American Stroke Association; meets on a monthly basis offering educational and emotional support for stroke survivors and their families.
o Animal Assisted Therapy provided five days a week through Therapet Animal Assisted Therapy Foundation.
o Strong relationship with Trinity Mother Frances Health System which leads to better continuity of patient care
o First in the area to provide state-of-the-art services such as Vital Stim Therapy and Interactive Metronome
o “Team Approach” to rehab and recovery, involving a physician specializing in physical medicine and rehabilitation; case manager; nursing staff; physical therapist; occupational therapist; speech therapist (as needed); dietitian; and pharmacist
o Focus on patient and family education as a part of treatment and recovery
o Known for ahigh level of clinical expertise
o Neuro-IFRAH trained clinicians and therapists
